At SFQ Solutions we strongly believe that each investigation needs to be approached from an organisational point of view. What does this mean?

We realise that human errors can and will happen. We cannot eliminate them entirely. What we can do instead, is developing an operational system that will help capturing these human errors.

Each investigation gives an opportunity for reviewing failed or absent defences that eventually allowed for an unsafe act to materialise as an accident. As part of the investigation process we propose to review any Organisational Influences, Unsafe Supervision and Preconditions for the Unsafe Acts, and identify both Active and Latent (Dormant) failures in the organisation structure and processes.

With this method you will not only minimise the risk of reoccurring accidents/incidents, but the overall organisational safety performance can be also improved.
